West Coast Instructions

To Become a Wholesale Member:

Step 1: We need a copy of your business license. If you belong to a public school, private school, or university martial arts club, we need a copy of the club's certificate from your school. If you belong to a YMCA/YWCA club, we need a letter from the program director on the institution's stationery.

When you have your business license, certificate, or letter, follow these steps:

Step 2: Download the following forms

You must have the free plug-in Adobe Acrobat to read the following documents.

• Credit Card Authorization Form
• COD Check Authorization Form
• Wholesale Application Form

Step 3: Fill out the forms with your information and choose one of the following options.

Step 3a: Mail the forms and a copy of your business license to: (See map below if you're unsure)

Step 3b: Fax the forms and a copy of your business license to:

Step 3c: You may also fill out and scan all of the documents above and email them to:
